Pop up to be enabled (Settings->search settings->content settings-Pop Ups->Allow Pop ups)
Step 1: Select the category from the given options.
step 2: Enter the temporary registration number you got.
Step 3: Enter the correct captcha code shown in the image.
Step 4: Press Show button
Step 5: Name and address of your establishment will be displayed.
Step 6: Press Print application button to download your application form
NOTE: Make sure that the pop up is enabled in your browser.
